We believe that in order for true healing to occur, a person needs to feel seen, heard and supported. Because of this, we created a medical practice that sees each patient as an individual, providing ample time and multiple touch points with their care team.
We are not your typical medical team, and this is not your typical medical experience.
As a patient of Collective Medicine, you'll have access to truly personalized medical care. which includes:
An extensive intake process
Our intake process is designed to gather ALL information relevant to your health, and includes a review of your previous labs, elaborate intake forms and a two hour session with your medical provider.
Based on the information gathered during your intake process, we will recommend a combination of diagnostic testing to go deeper, providing us with data to understand your baseline, how to optimize your care and as a tool to assess progress.
A customized health plan
The creation of your Health Plan begins with your initial visit, and will continue to evolve throughout your Care Plan. This includes nutritional therapies, targeted supplementation, lifestyle modifications and other interventions, continuously monitored and supported by your care team.
Our health coaching sessions help you implement your customized Health Plan, guiding you in making successful dietary shifts and incorporating lifestyle changes that will have the greatest impact on your healing journey.
Each care plan includes multiple follow-up sessions with your medical provider and health coach, allowing you to share feedback and providing you with support that keeps you accountable.
Ongoing support & accountability
We are here to support you. We provide unlimited messaging between sessions and offer ways to continue working with us, longer term.
